Steps in Board Rating Application, PRC License Application and Oath taking Application for June 2009 NLE
Board Rating Application:
1. Bring 1 valid id
2. Go to PRC (fifth floor)
3. Get your rating
PRC License Application:
1. Go to PRC
2. Get and Fill-out Oath Form
3. Bring/Buy Recent Cedula
4. Bring/Buy 1 SHORT brown envelope and label it with your name and profession
5. Have your oath form notarized (40php) and stamped (21php)
6. Place the accomplished oath form and the cedula inside the envelope together with:
a. 2 passport size colored pic, white background and with name tag
b. 1 1×1 pic white background and with name tag
7. Submit them at the fifth floor of PRC on the following schedule:

FOR NEW AND UPDATED SCHEDULE PLEASE CLICK HERE.
8. Pay php 1050.00
9. Get the stub for the claiming of license.
Oath Taking Application:
BON Advisory: RN Oathtaking Instructions
1. Go to Philippine Nurses’ Association (PNA), FT. Benitez St., Malate, Manila starting July 29.

2. Pay the following amounts:
a. Initial registration fee for PNA membership: php 400.00
b. Tickets: php 350.00 – inductee
php 250.00 – guest
Requirements:
(No update: Just bring the documents for your PRC license application just to make sure, 1-2 valid ID’s, NOA and Board Rating)

Maraming Maraming Salamat po sa LAHAT LAHAT ng info kasi nung Monday nalilito ako kung anu na ba ang dapat kong asikasuhin. Natapos ko din ng maayos yung Board Rating sa 5th floor pati yung PNA Membership at OathTaking Tickets. Wait ko na lang ang August 10 para sa Registration ng License. Congrats New RNs!

I’m a board passer from Ateneo de Zamboanga University, is it possible to have my oath here together with the other MANILA schools at SMX? If yes, when could our schedule be?

I wonder if you can help me. For the board rating, can you only get it from where you took the test (ex Baguio) or its possible to get if from any PRC office?

@havana- hi! yes, you can get your board rating at any PRC office… they will print it out for you, just make sure you bring valid ID…

@shane benedicto- i believe the schedule is per school basis, you can check it out in this site… If you really want to have your oath taking here in manila, you can register and get your ticket at PNA and by then you will now your schedule…

Dear Moderator,
The schedule for the initial registration of new nurses was updated las August 10, 2009. August 21 is a holliday. There is no registration on August 31. Kindly refer to PRC for the updated schedule of registration. Thank you and more power to you!
